Instrument request form glitch

" problems with the instrument request form: 1) that you have to select a US state, even if the request is from a foreign country. 2) The equipment request page has a power-supply section. I thought perhaps one should delete the classes that were not applicable. So I selected and hit delete, and the entire form disappeared, nd I had to start al over again. This selection should be radio buttons -- that is, more clearly a list. It looks like a place where the user should alter or type in info. Fixed - Dave Thomas
